			We thank our doctors and referral sources for sending your patients to Health Ex Physical Therapy. We appreciate your understanding of our approach to offering lasting pain relief and rehabilitation and improving your patients' quality of life. If you are new to the Health Ex philosophy, please take a moment to learn more about our services and features by following the links at left. We have a clinic on the Westside of Jacksonville and one on Fleming Island. While your patient is at Health Ex, we will send progress reports to you and maintain direct communication between our staff and your office. In addition, our staff will bill the patients' insurance company. You may fax your referral to either of our two locations Westside Jacksonville at (904) 772-8575 or to our Fleming Island office at (904) 264-0012. We will notify the patient and set up a convenient appointment time.
